- W2979238428 abstract "Introduction: The Centers for Medicaid and Medicare Services have recently focused on outcome measures for common acute medical diagnoses including acute myocardial infarction (MI), congestive heart failure (CHF), pneumonia, hip and knee replacements. However, the economic and clinical impact of inflammatory bowel disease (IBD) in patients in these conditions has not been studied. We investigated length of stay (LOS), cost of hospitalization, and mortality in IBD patients admitted for these diagnoses compared to controls using the Healthcare Cost and Utilization Project National Inpatient Data (NIS). Methods: NIS 2005-2011 was used for the study. Continuous variables were reported as mean ± standard deviation. Normality assumptions for continuous variables were assessed using the Shapiro-Wilk test. Continuous variables were compared by two-sample T-tests or Wilcoxon rank-sum tests. Categorical variables were analyzed by Chi-square or Fisher's exact tests. Propensity score matching method is widely used in observational studies to reduce selection bias. To fairly compare the outcomes between IBD patients and controls, we conducted 1:3 propensity score matching using a matching algorithm based on patient age, gender, race, number of co-morbidities, total numbers of procedures, insurance, income quartiles, hospital bed size, region and hospital teaching status. The McNemar test was used to compare binary outcomes between matched groups. Statistical analyses were performed on SAS 9.3 (Cary, NC). Matching to control groups was repeated for Crohn's disease (CD) and ulcerative colitis (UC) patients individually for each of the aforementioned primary diagnoses. Results: IBD patients hospitalized for acute MI, CHF, and knee replacements did not have significant differences in LOS or costs compared to controls (table 1). IBD patients with pneumonia had a significant increase in LOS and costs compared to controls. IBD patients hospitalized for hip replacements showed a signficantly increased LOS. Statistically decreased death for acute MI and pneumonia was seen in patients with CD, (MI 4.0% death in CD vs. 6.1% in controls, p < 0.001; pneumonia 2.6% death in CD vs. 4.3% in controls, p= < 0.0001), (table 2). UC patients with MI had decreased death (MI 4.2% death in UC vs. 6.1% in controls, p < 0.05). UC patients had increased LOS, costs and death for CHF (table 3). Conclusion: The co-diagnosis of IBD impacts hospitalization outcome for these common acute medical conditions.Figure 1Figure 2Figure 3" @default.
