- W2979245816 abstract "Purpose: Chromogranin A (CgA) a marker of neuroendocrine tumors is shown in colorectal adenocarcinoma but its expression is not well documented in serrated polyp and tubular adenoma of colon. Ki67, a proliferative marker is expressed in normal colonic crypt and limited data has suggested its presence in serrated polyp and tubular adenoma. Our purpose was to study expression of Chromogranin A and Ki-67 in above mentioned colorectal pathology. Methods: We randomly selected histology slides of 40 serrated polyps and 10 tubular adenoma from year 2002-2003 at John Dempsey hospital. Immunehistochemical analyses were performed and expression of CgA and Ki67 were studied and compared with 5 normal colon specimens acting as control. Neuroendocrine cells are present at the base of colonic crypts. We hypothesized based on previous study (unpublished) that serrated polyp should express CgA as they develop from base of crypt and should lack expression in tubular adenoma as they develop from the upper third of colonic crypt. Results: Normal colonic crypts specimen expressed CgA at base and were absent in upper third of colonic crypt. Serrated polyp expressed CgA similar to that observed in normal crypts and the findings were consistent throughout 40 cases of serrated polyp. On the contrary, tubular adenomas were mostly devoid of CgA. This correlates with our hypothesis.Ki-67 was expressed in base of crypts and was absent in upper third of normal colonic crypt. In serrated polyp, Ki-67 was predominantly expressed in basal crypts with a decreasing gradient from base to upper third i.e. the presence of Ki-67 gradually diminished from base to upper portion of crypt. Tubular adenoma expressed Ki-67 at surface of crypt with lack of expression at the base suggesting higher proliferating activity on surface of colonic crypts. Conclusion: Neuroendocrine markers are not well studied in serrated polyp. Our findings indicate the presence of CgA in crypts bases in serrated hyperplastic polyp compared to near lack of CgA in tubular adenoma. Ki-67 is similarly expressed in mid to lower third colonic crypt in serrated polyp compared to predominant expression on crypt surface in tubular adenoma and is consistent with previous studies. Further studies are needed to understand the significance of variation in expression of neuroendocrine and proliferative markers in development of colorectal polyps." @default.
