- W2979253458 abstract "Purpose: To describe the clinical and pathological outcomes of Hispanic patients with ulcerative colitis (UC) who underwent ileal-pouch anal anastomosis (IPAA) at the University Hospital 1994-2010. Methods: Our study consists of retrospective data gathering to identify and describe pouch outcomes in our population of Hispanic patients with UC. Eighty-eight patients diagnosed with UC 1970-2008 who underwent IPAA 1994-2010 and who are followed in our clinics were identified. Medical charts of a cohort of 61/88 patients who had previously consented to participate in the IBD registry and who were currently enrolled in a pouch dysplasia surveillance protocol were analyzed. A separate analysis was performed on a cohort of 12/88 patients who developed adverse pouch outcomes. Results: The population consists of 48% female and 52% male. Thirteen of sixty-one (21.3%) patients had at least one episode of acute pouchitis and chronic pouchitis developed in 16.3% (10/61). Irritable pouch syndrome was identified in 8.1% patients (5/61). Cuffitis was present in 42.6% of patients endoscopically (26/61) and a normal pouch was observed in 47/61 (77%) patients. Surgical complications such as stricture and pouch leaks were present in 37.7% and 3.2% respectively. Pouch dysplasia was diagnosed in 3.2% (2/61). In the second cohort of 12/88 (14%) of UC patients with adverse pouch outcomes, chronic continuous refractory disease was the sole indication for IPAA. The median interval between IPAA and development of complication was 4 years (range 0.3 to 9.6 years). The most common complications were stricture and/or fistula formation (9/12), being de novo Crohn's Disease of the pouch (4/12) the leading cause of pouch failure, followed by surgical complications (3/12). Pouch defunctionalization was required in 83% (10/12) patients with adverse pouch outcomes and 11.3% (10/88) of the total population. Conclusion: It has been previously described that Puerto Ricans with Ulcerative Colitis have more extensive disease and a higher rate of colectomy. In spite of this, our patients have a higher prevalence of normal pouches when compared to previously described series of Hispanics." @default.
