Because many hospitals have no formal mechanism for written translations, many providers resort to the use of automated translation software, such as Google Translate. A recent study using professional translators to evaluate discharge instructions in Spanish and Chinese suggested that accuracy rates of Google Translate were high. The primary objective of this study is to perform a pragmatic assessment of the accuracy of Google Translate for the written translation of commonly used ED discharge instructions in seven of the most commonly spoken languages in our patient population using non-medical volunteers. The secondary objective is to compare the performance of Google Translate between languages. We performed a prospective assessment of the accuracy of Google Translate for 20 commonly used ED discharge instruction phrases, as evaluated by a projected convenience sample of 14 native speakers of 7 commonly spoken languages (Spanish, Chinese, Vietnamese, Tagalog, Korean, Armenian, and Farsi) in an urban emergency department. We collected demographic data on the participating subjects, including years in the US, self-reported English and target language proficiency, and acculturation level. We measured translation accuracy using a previously validated matrix for scoring machine translation, which included a five-point Likert scale measuring fluency, adequacy, meaning, and severity, in addition to a dichotomous variable of whether the statement conveyed the overall meaning. The primary outcome of overall accuracy was the total number of statements correctly translated compared to the total number of statements, in aggregate and for each language. We compared the accuracy rate by language using proportions. Subcategories of fluency, adequacy, meaning, and severity were reported as a mean Likert scale score. To date, study subjects have evaluated 200 total discharge statements translated by Google Translate. Volunteers were 60% female and spoke Chinese (3), Spanish (2), Armenian (2), Korean (1), Farsi (1) and Tagalog (1). Mean years living in the U.S. was 19 (range 3-45). All self-reported that they spoke English ‘Well’ or ‘Very Well,’ and their native language ‘Very Well’ and had high levels of acculturation. The overall accuracy of the translated statements was 78.5% (157/200) but varied greatly between languages with Tagalog having the highest overall accuracy rate (95%) and Armenian the lowest (55%). Mean Likert scores were high for fluency (4.1), adequacy (4.2), meaning (4.0) and severity (4.1). However, the mean Likert score of these subcategories was consistently lower for Farsi and Armenian as compared to the other languages. Google Translate for discharge instructions in the ED is inconsistent between languages with accuracy rates ranging from 55- 95%. Providers should be cautious when using automated translation software for written communication with LEP patients. Other solutions are necessary for written translation of ED discharge instructions in languages of lesser diffusion." @default.
