- W2979672214 abstract "Hospital-based expenditures account for one-third of U.S. health care costs. Our prior work shows a 20% increase in ED visits and 10% decline in ED admission rates from 2006-2014; however, the drivers of this shift from inpatient to outpatient care are unknown. We aim to describe condition-specific trends and hospital-level variation in ED admission rates. We retrospectively analyzed all adult ED visits to acute care hospitals from 2006-2014 in the National Emergency Department Survey. We excluded visits with a disposition of left without being seen, left against medical advice, transferred, or died. We describe trends in most frequently admitted conditions and use logistic regression to compare the odds of ED admission from 2006 to 2014. We adjusted for patient (age, sex, insurance, income, Elixhauser comorbidity score) and hospital (teaching status, geographic location, region, annual ED visit volume) characteristics. There was a decrease in the median hospital all-cause ED admission rate from 19.4 to 17.5% and increase in interquartile range (IQR) from 11.8 to 13.8%. The conditions with the greatest absolute change in ED admission rates over the time period were appendicitis (79.5 to 59.2%) and coronary heart disease (81.5 to 61.2%), while the conditions with the greatest relative change in ED admission rates were chest pain (21.7 to 7.5%) and syncope (28.9 to 13.8%). The conditions with the greatest adjusted change in likelihood of ED admission were nonspecific chest pain (OR 0.16, CI 0.14-0.19) and congestive heart failure (OR 0.17, CI 0.15-0.2). For chest pain and syncope, the conditions with the greatest relative reduction in ED admissions, we observe a decrease in hospital-level variation in ED admission rates, with a reduction in IQR from 21.6 to 6.4% for chest pain, and 23.6 to 12.5% for syncope. For congestive heart failure and pneumonia, the two most frequently admitted conditions, there appears to be little change in the hospital-level variation and IQR over time. After adjusting for patient and hospital factors, the odds of overall ED admission in 2014 compared to 2006 was 0.49 (CI 0.45, 0.52). The odds of admission after an ED visit for any condition were over 50% lower in 2014 compared to 2006 after adjusting for patient and hospital characteristics. If the 106 million ED visits made in 2014 had an ED admission rate similar to that of 2006, an additional two million hospitalizations would have been expected that did not occur—representing a savings of at least $20 billion, based on an average cost of $10,400 per hospitalization. The decline in ED admissions appears to be driven by a select number of high variation, “ED-sensitive” conditions such as chest pain, where there is clinical equipoise in the decision to admit a patient. The decline in ED admissions was driven by select hospitals with substantially lower ED admissions over time, as opposed to equally distributed reductions across all hospitals, suggesting increased variation and opportunities for quality improvement.
