- W2980486580 abstract "Increasing evidence suggests that depressive symptoms may be early manifestations of Alzheimer's Disease (AD), but the relationship of specific depressive symptoms to AD pathology remains unclear. The current study was carried out to determine if specific depressive symptoms were associated with amyloidosis and cognition in cognitively unimpaired older adults. In this cross-sectional sample from A4 screening, 4,486 cognitively unimpaired participants without depressive illness (age:71.3±4.7, 41% male) underwent amyloid (florbetapir) PET; 1,323 (29.5%) were classified as Aβ+. Depressive symptoms (GDS-15) and cognition (Preclinical Alzheimer Cognitive Composite (PACC) were assessed prior to PET. Individual GDS items with endorsement frequency of >=10% in each Aβ group were identified and then entered as outcomes in separate logistic regression models with amyloid status (Aβ+/Aβ-) as the main predictor and age, sex, and education as covariates. To determine if these items were associated with cognition, they were entered as predictors in linear regression models with PACC as the outcome with the covariates above. Three GDS-15 items were endorsed by >10% of each Aβ group: ‘having more problems with memory than most’; ‘preferring to stay home rather than trying new things’; and ‘not feeling full of energy’. When these items were entered as outcomes in separate regression analyses with predictor amyloid, only ‘having more problems with memory than most’ was positively associated with amyloid status (Aβ+/Aβ-) (OR: 1.45, 95% CI [1.21,1.74], p<0.001); females endorsed this item more (OR: 0.79, 95% CI [0.66,0.95], p=0.01). In models with outcome PACC, of the three GDS items, only “more problems with memory” was negatively associated with PACC (estimate: −0.79, p<0.001). In a preclinical AD trial screening sample with restricted depression severity, a depression item probing memory concern was associated with greater amyloid and worse cognition, while other depressive symptoms were either endorsed infrequently or not associated. This suggests that depression assessment at screening may not add sensitivity to subjective cognitive report questionnaires. Future longitudinal investigation at different stages of disease progression and in samples with non-restricted depression severity is needed to better understand the temporal sequence of depressive symptom dimensions in preclinical AD." @default.
