- W2980608022 abstract "Current brief episodic clinical assessments such as cognitive tests or functional scales are inherently variable and not ecologically valid. Data captured at home in real-world settings using remote sensing and digital technologies to provide indicators of objective change (“digital biomarkers”; DBM) can improve upon conventional methods. However, biological confirmation of DBM signals relative to neuropathologies that may be driving DBM readouts has been lacking. Participants followed longitudinally in ORCATECH (Oregon Center for Aging & Technology) aging studies were monitored for over a decade with an unobtrusive activity sensing platform installed in their homes. Algorithms run on the continuously collected passive sensor data generated daily functional metrics for each volunteer: mobility (walking speed), cognition (computer use time), socialization (time out of home), and sleep (total sleep time). The activity data analyzed came from the last available one-year period from forty-one volunteers who were followed until death and had brain autopsy with post mortem evaluation according to the UDS National Alzheimer's Coordinating Center coding system. Forty-one subjects were available for analysis. Cohort mean age at death was 92.2 years; median duration from DBM home sensing completion to death (and autopsy) was 1 23 of 41 died while their home was sensored) median MMSE before death was 27, 5.9). The four DBMs showed consistent patterns relative to both Braak stage and plaque score severity (see figures). A composite DBM measure including the four activity domains (mobility, cognition, socialization and sleep) was constructed.
