- W2980701701 abstract "American Indians (AI) have higher rates of dementia than Whites. The high prevalence of cerebrovascular risk factors, including metabolic syndrome (MetS), in AI populations may contribute to this disproportionate risk: cerebrovascular disease is associated with cerebral ischemia and cognitive decline. MetS can be represented by waist-to-hip ratio (WHR), a non-invasive, easily accessible measure of abdominal obesity, and the MetS marker most strongly associated with cerebral ischemia. Despite disproportionate vascular disease and dementia burden, AI populations are underrepresented in cognitive aging research and cerebrovascular models of risk and disparity have rarely been explored. We examined the association between WHR and both total white matter lesion volume (TLV) and cognitive performance in AI and non-Hispanic white older adults. The study sample included 599 (White:93%, AI:7%) participants enrolled in the Wisconsin Alzheimer's Disease Research Center who completed cognitive testing and provided anthropometric and imaging data on an annual or biennial basis. We utilized a causal pathways approach to test whether WHR mediates the relationships between AI race (self-reported) and (a) executive function as measured by Trails Making Test-B (TMT-B) completion time, and (b) TLV. For both models, we controlled for age, education, gender, and APOEe4 status. Sample characteristics by race are represented in Table 1. Mediation analysis results are illustrated in Figures 1 and 2. AI race was associated with higher WHR (B=0.09; p<0.001) and poorer TMT-B performance (B=19.59; p=0.001). However, WHR was not associated with TMT-B performance (B=11.83; p=0.513). In our second model, neither AI race (B=−2.69; p=0.432) nor WHR (B=16.13; p=0.146) was associated with TLV. Our findings suggest that WHR does not mediate the relationships between (a) being AI and TMT-B, or (b) being AI and TLV." @default.
