- W2980782017 abstract "Working in mentally challenging occupations may increase cognitive reserve (Stern, 2002, 2009) and protect against dementia in late life. However, there has been little evidence regarding the association between mentally challenging occupations and dementia risk in a racially diverse population. The aims of the current study are i) to investigate whether working in mentally challenging occupations is associated with decreased risk of developing dementia in a racially diverse, community-based sample and, ii) to examine whether the effects of mentally challenging occupations are equivalent between non-Hispanic Whites (nHW) and African-Americans (AA). The sample consisted of 1,055 individuals (65.6% nHW, 28.5% AA, 5.9% others; mean age=78.6±5.3; 61.5% female) enrolled in the Einstein Aging Study, a longitudinal study of older adults aged 70+. Occupation information of each participant was collected at baseline and was linked with a mental work demands (MWD) score from the U.S. Dictionary of Occupational Titles as described by Roos & Treiman (1980). We defined low MWD as the bottom quintile and high MWD as the remaining quintiles. Cox proportional hazards models were used to evaluate the association of MWD with the risk of dementia. The high MWD group had more years of education, was more likely to be nHW, and had incomes above the poverty level. Results from Cox regression indicated that individuals with low MWD, compared to those with high MWD, showed a significantly higher likelihood of incident dementia (HR=1.65; 95% CI=[1.12-2.25])(Table 1). When further stratified by race, low MWD was significantly associated with higher risk of developing dementia only for AA (HR=3.07; 95% CI=[1.60-5.90]) but not for nHW (HR=1.12; 95% CI=[0.63-1.98])(Table 2). The difference in the effect of MWD on incident dementia between nHW and AA was significant (p<.05).
