- W2980851552 abstract "During the course of dementia, most develop some type of neuropsychiatric symptoms (NPS), which result in high family caregiver burden, increase caregiver stress, impaired quality of life, and may lead to institutionalization. The authors examined the link between presence of clinically significant neuropsychiatric symptoms and progression to death among community dwelling home bound dementia patients referred for physician home visit with regular follow up for up to 7 years. Between 2011 and 2018, we have received 6657 referrals and extracted patients that were seen at home or a non medical facility. Non medical facility is defined as a group home or individual home without a licensed staff requirement. Residents of nursing home or medical home settings that requires licensed nursing staff are excluded from this study. Subjects were visited monthly by a physician for their medical needs including behavioral monitoring and medication management. Out of the total of 1598 patients that are residents of non medical homes, 173 of these patients referred were due to NPS. Among the NPS group, 68.8% of the patients are women, 84.4% are >75 yo, average age of death is 86.19 years. Average time to death from referral is 713 days (median of 532 days). (Table 1) 86.7 % of NPS patients are on acetylcholinesterase inhibitors, psychotropic, antidepressant, anxiolytic or combination of medications. In the span of 7 years, 74.1% of the NPS group and 55.28% of the overall group have died. Mortality rate among dementia with NPS subjects are significantly higher (1.8 - 2.3 times) and remained high during the course of 7 year follow up. (Figure 1)" @default.
