- W2980854537 abstract "Dementia represents one of the greatest causes of disease burden; however, little is known about the cost of the disease. We estimated the total, annual, and net cost (value of informal care, out-of-pocket expenditures, Medicaid long-term care facility expenditures, and Medicare expenditures) of dementia in the United States from a societal, health system, and family perspectives. To estimate the cost of dementia, we developed an evidence-based mathematical model to simulate disease progression for newly diagnosed individuals using three important clinical features - cognition, function, and behavior. Disease trajectories were estimated from regression analyses of the Uniform Data Set of the National Alzheimer's Coordinating Center. Clinical features affected transitions between place of residence (community or long-term care facility) and insurance status (Medicare-only or Medicare-Medicaid). Data from the Health and Retirement Study linked to Medicare were used to predict costs associated with dementia clinical features. At any time, a person with dementia could die of dementia-related or other causes. Using modeling, we evaluated the lifetime and annual costs among those with dementia. We also compared costs between simulated individuals with and without dementia clinical features (i.e., net dementia cost). The discounted total lifetime (mean age of dementia onset 83 years) cost of care for a person with dementia was $321,780 (2015 dollars). Medicaid and Medicare (i.e., health system perspective) accounted for 30% ($96,630) of total costs. Families incurred 70% of the total cost burden ($225,140). Total annual cost peaked at $89,000 in the 6th year post onset. Costs for a person with dementia over a lifetime were $184,500 greater (86% incurred by families) than for someone without dementia. Dementia greatly increases the lifetime costs of care. Most dementia costs are borne by families. Services should be widely implemented to support family caregivers of persons with dementia." @default.
