- W2980889039 abstract "High resting heart rate (RHR) and low heart rate variability (HRV), indicators of cardiac autonomic imbalance, have been associated with multiple risk factors for dementia including cardiovascular disease, diabetes and hypertension. Emerging evidence shows associations between autonomic imbalance and poor cognitive function, yet whether autonomic imbalance predicts dementia incidence is unknown. The study sample includes participants from the Framingham Offspring cohort who were 18 years or older and had information on RHR and HRV at baseline (exam 3; 1983-1987). Those with prevalent atrial fibrillation, heart failure, dementia, stroke or other neurological condition at baseline were excluded. RHR and HRV were derived from ECG at baseline. HRV was assessed through the SD of normal-to-normal intervals (SDNN) and the square root of the mean of the squared differences (RMSSD) from 2-hour Holter monitor data. Participants were followed-up for dementia incidence starting from exam 7 (14.1±1.0 years after baseline) for a mean duration of 10 years. Cox regression models were used to assess the link of RHR and HRV with dementia risk while adjusting for multiple potential confounders, including physical activity and use of antiarrhythmic medications. Of the 1,581 participants assessed at baseline (mean age=55±6y, 46% men), 133 developed dementia. Overall, autonomic balance was not associated with dementia risk. However, age at ECG as well as sex and use of β-blockers significantly modified this association. When HRH and HRV were assessed at ages ≥60 years, each increase of 10 beats-per-minute in RHR was associated with increased dementia risk (HR (95% CI)=1.25 (1.01,1.54)) and increases of 1SD in SDNN and RMSSD were significantly associated with decreased dementia risk (HR (95% CI)=0.48 [0.29,0.80] and 0.32 [0.15,0.71], respectively). Moreover, increase in 10 beats-per-minute of RHR was associated with increased dementia risk among females (HR (95% CI)=1.25 (1.01,1.54)) and 1SD increase in RMSSD was associated with decreased dementia risk among users of β-blockers (HR(95% CI)=0.14 (0.02,0.81)). These findings suggest that measures of autonomic balance assessed in middle and old ages may serve as novel predictors of dementia incidence. Research is warranted to explore the possible role of autonomic imbalance in dementia pathophysiology." @default.
