- W2981185221 abstract "SESSION TITLE: Monday Fellow Case Report Posters SESSION TYPE: Fellow Case Report Posters PRESENTED ON: 10/21/2019 02:30 PM - 03:15 PM INTRODUCTION: Fibroepithelial polyps are common benign lesions found on the skin and genito-urinary tract. Rarely does it occur in the tracheobronchial tree with only a few case reports published in the literature. Histologically it consists of a fibrovascular stroma covered by respiratory epithelium. We present a case of a middle-aged man presenting with pleuritic chest pain found to have a fibroepithelial polyp diagnosed by fiberoptic bronchoscopy. CASE PRESENTATION: A 43 year-old man, former smoker, presents to the office for pleuritic chest pain and shortness of breath for one year. He had no other significant complaints but did report a family history of lung cancer. Physical exam and laboratory studies were non-revealing. Imaging studies including computer tomography (CT) of the chest revealed a bi-lobed soft tissue mass in the superior segment of the left lower lobe, a density in the right main-stem bronchus with enlarged R4 and 11L lymph nodes. Patient underwent bronchoscopy with endobronchial biopsies and bronchial alveolar lavage. Biopsies obtained revealed that the left lower lobe lesion was a hamartoma and the right upper lobe mass was a fibroepithelial polyp. The patient was scheduled for short-term follow-up however patient did not follow-up. Patient returned 2 years later after presenting to an outside hospital with pleuritic chest pain which prompted a CT angiography of the chest to be performed revealing new pulmonary nodules and enlargement of the known left lower lobe nodule and mediastinal lymphadenopathy. DISCUSSION: Endobronchial fibroepithelial polyps are a rare entity representing 1.9% of all pulmonary tumors. Although they are classified as a neoplasm, many believe they are more inflammatory than truly neoplastic in nature, which is supported by the fact there are fewer findings of fibroepithelial polyps since the introduction of antibiotics. The etiology is unclear, however it is though to be due to chronic inflammation, often seen in smokers. Other potential irritants have been implicated including foreign body aspiration, prolonged mechanical ventilation, asthma, chronic sinusitis, and mycobacterial infections. It can manifest as wheezing, cough, dyspnea, hemoptysis, or rarely as pleurisy as seen with this patient. Since the tumor is covered with normal mucosa, bronchial washings and brushings are often non-diagnostic. Biopsy is often difficult to obtain because of movement of the polyp and therefore these lesions are often removed to make the diagnosis. CONCLUSIONS: Bronchial fibroepithelial polyp is a rare entity however it should be on the differential if an endobronchial lesion is seen on imaging studies. Endobronchial resection is the mainstay of treatment and surgery is rarely required. Reference #1: Wartmann CT, Fernandez D, Flores RM.
