- W2990432017 abstract "BACKGROUND: Air pollution may promote systemic inflammation and insulin sensitivity leading to pathophysiologic changes in glucose metabolism. While some evidence suggests nitrogen dioxide (NO2) and particulate matter (PM2.5) may be linked with type II diabetes, studies have been inconsistent. Whether these exposures are associated with increased glycosylated hemoglobin (HgbA1c) levels remains unclear. METHODS: We used multilevel regression models to evaluate the association between long-term average pollutant levels and prevalence of diabetes and increases in HgbA1c among 4,121 individuals over 57 years of age enrolled in the National Social Life, Health, and Aging Project. We calculated PM2.5 estimates on a six-kilometer grid covering the conterminous U.S. using spatio-temporal models, and nitrogen dioxide estimates based upon the EPA’s Air Quality System measurements. HgbA1c levels were measured from dried blood spots using the Roche Unimate method. All health effect models were adjusted for age, sex, BMI, smoking status, race, household income, education level, neighborhood socioeconomic status, diabetic medication use, and distance the participant lived from a major roadway. RESULTS: An IQR increase in the one-year moving average of PM2.5 was positively associated with diabetes prevalence (OR 1.184, 95% CI 1.080, 1.297) and an increase in average HgbA1c of 0.048%, (95% CI: 0.015, 0.081). Similarly, an IQR increase in NO2 was associated with diabetes prevalence (OR 1.222, 95% CI: 1.070, 1.394) and an increase in average HgbA1c of 0.071% (95% CI: 0.030, 0.112). These associations were more pronounced among obese individuals. Conclusions: In a cohort of older men and women in the United States, PM2.5 and NO2 exposures were significantly associated with prevalence of diabetes and increased HgbA1c levels. If causal, these associations suggest that air pollution could be an important and ubiquitous risk factor for abnormal glucose metabolism and diabetes." @default.
