Na odontologia a dor resultante de lesao tecidual e uma constante no atendimento clinico, podendo ser evitada com o uso da administracao de anestesicos locais (AL) que podem ser administrados com ou sem vasoconstritor (VC), o qual pode ser derivado das aminas simpatomimeticas ou analogo da vasopressina. A utilizacao de vasoconstritor pode causar varios efeitos adversos, dentre os quais podemos destacar os relacionados ao sistema cardiovascular (pressao arterial e o ritmo cardiaco), esses efeitos hemodinâmicos sao causados pela acao direta dos AL sobre o musculo liso ou musculo cardiaco e sao dose-dependentes. Tais efeitos sobre o sistema cardiovascular podem comprometer a fisiologia do sistema nervoso central, uma vez que prejudicam o fornecimento de sangue, nutrientes e gas oxigenio, ocasionando disfuncao na fosforilacao oxidativa nessas celulas e assim diminuindo a sintese de ATP, substância quimica responsavel pelo fornecimento de energia para a celula. A associacao de AL com VC em pacientes submetidos a procedimentos cirurgicos odontologicos pode causar elevacao da pressao arterial, devido a vasoconstricao e aumento da resistencia vascular, assim como diminuicao da frequencia cardiaca, provavelmente, pela ativacao de mecanismo homeostatico relacionado aos barorreceptores e arritmias cardiacas pelo bloqueio de canais de sodio localizados nos cardiomiocitos. No entanto, vale a pena destacar que, na maioria dos casos, a associacao de AL com VC nao causou alteracao significativa nos parâmetros cardiovasculares. CARDIOVASCULAR ALTERATIONS CAUSED BY THE ASSOCIATION OF LOCAL ANESTHETICS WITH DRUGS THAT CAUSE VASOCONSTRITION IN DENTAL PROCEDURES Pain is the result of peripheral nerve impulses activated by local stimuli and may also be the result of emotional reactions. In dentistry the pain resulting from tissue injury is a constant in clinical care, and this can be avoided through the use of local anesthetic (LA) administration. These anesthetics may be administered with or without vasoconstrictor, which may be derived from sympathomimetic amines (VDAS) or vasopressin analog (VAV). The use of a vasoconstrictor can cause several adverse effects, among which we can highlight those related to the cardiovascular system (blood pressure and heart rate), these hemodynamic effects are caused by the direct action of LA on the smooth muscle or heart muscle and are dose- dependents. Such effects on the cardiovascular system may compromise the physiology of the central nervous system, since they impair the supply of blood, nutrients and oxygen gas, causing dysfunction in oxidative phosphorylation in these cells and thereby reducing the synthesis of ATP, the chemical responsible for the supply of Energy to the cell. The association of AL with VAV in patients submitted to dental surgical procedures generally promotes an elevation of blood pressure and vascular resistance, as well as a decrease in heart rate, probably due to the activation of a homeostatic mechanism related to baroreceptors. However, the association of AL with VDAS, in general, did not cause a significant change in cardiovascular parameters." @default.
