- W2998678609 abstract "ACOG recommends the yearly influenza vaccination for all pregnant patients as it reduces maternal and fetal mortality and morbidity. However, for the 2017-2018 influenza season, only 49.1% of pregnant patients were vaccinated. We examined the cost-effectiveness associated with vaccinating all pregnant patients against influenza. We designed a cost-effectiveness model using TreeAge to compare outcomes in pregnant patients who received the inactivated, trivalent influenza vaccine to those who did not. We used a theoretical cohort of approximately 4 million patients, the number of births in the United States per year. We assumed that H1N1 and A or B type influenza were of equal prevalence based on seasonal variation from the past ten years. Our outcomes include acquiring H1N1 or A/B type flu, maternal death, stillbirth, infant death, preterm birth, and cerebral palsy in addition to cost and quality-adjusted life years (QALYs). Probabilities were derived from the literature and QALYs generated at a discount rate of 3%. Sensitivity analyses were performed to assess the robustness of our model. Results: For a one-year theoretical cohort of 4 million patients, the influenza vaccine resulted in a decrease of 5,124 stillbirths, 8,602 maternal deaths, 561 infant deaths, 56,347 preterm births, and 492 cases of moderate cerebral palsy (Table 1). Vaccination saved 17.5 billion dollars and increased QALYs by 422,604. Univariate sensitivity analysis demonstrated that the vaccine is cost saving until vaccine price passes $4500, far from the current price (Figure 1). In addition, we used sensitivity analysis to vary seasonal ratios of H1N1 to A/B type influenza. The vaccine was cost saving and increased QUALYs for any H1N1 to A/B type influenza ratio including when either H1N1 or A/B type was absent. In our cohort, the influenza vaccine saved 17.5 billion USD per year and increased QALYs by 422,604.
