- W2999504414 abstract "A 27-year-old man presented to the emergency department with penile pain and inability to urinate. He endorsed inserting 2 objects into his urethra hours before and advancing them with a pen. He had been unable to urinate since. Physical examination result was positive for palpable masses along the ventral aspect of the penis. Bedside renal ultrasonographic result was positive for a distended bladder, but negative for hydronephrosis or intravesicular foreign bodies. Bedside penile ultrasonography was then performed. Urinary tract obstruction by urethral foreign bodies with urinary extravasation. Bedside penile ultrasonography was positive for 2 echogenic, shadowing foreign bodies, with signs of free extraurethral urine (Figure 1, Figure 2 to 3 and Videos 1 and 2). Urology was consulted and extracted the foreign bodies at the bedside. The patient was then able to void without difficulty.Figure 2Long-axis, transpenile, curvilinear ultrasonography along the dorsal penis, demonstrating proximal (arrow) and distal (arrowhead) echogenic, shadowing foreign bodies.View Large Image Figure ViewerDownload Hi-res image Download (PPT)Figure 3Extracted foreign bodies (earbud and toothpaste cap), arranged as visualized on ultrasonography.View Large Image Figure ViewerDownload Hi-res image Download (PPT) Urethral foreign bodies are a relatively rare clinical occurrence that can result in genitourinary trauma, infection, and obstruction.1Palmer C.J. Houlihan M. Psutka S.P. et al.Urethral foreign bodies: clinical presentation and management.Urology. 2016; 97: 257-260Abstract Full Text Full Text PDF PubMed Scopus (34) Google Scholar Imaging is often warranted if the diagnosis is not readily apparent according to patient history or examination result, and to guide management. Imaging modalities vary according to the location and radiolucency of the objects. Ventral penile and dorsal transpenile ultrasonography was used in this case for foreign body localization and identification. Bedside ultrasonography can be used to evaluate the size, shape, and orientation of urethrovesical foreign bodies, as well as to evaluate for signs of significant renal obstruction or urine extravasation. Bedside ultrasonography can also facilitate suprapubic bladder aspiration or catheter placement if intervention is expected to be delayed.2Aguilera P.A. Choi T.
