Erweiterte Ureteren vom Typ I und II gemäß der Klassifikation nach Pfister und Hendren normalisieren sich im Verlauf der ersten Lebensjahre spontan, während stark dilatierte Systeme des Pfister Hendren Typ III keine Maturationsfähigkeit haben. Bezogen auf diese besondere Gruppe der Typ III Megaureteren empfehlen wir ein 2-zeitiges operatives Vorgehen: Erstens legen wir in einer frühzeitigen Operation eine splintfreie temporäre Ureterocutaneostomie zur Desobstruktion an und zweitens führen wir den eigentlichen Korrektureingriff mit zeitgleicher Aufhebung des inkontinenten Harnleiterhautstomas um den ersten Geburtstag durch, ein Zeitpunkt, zu dem die Blasenfunktion gereift ist. Durch dieses Vorgehen sind sonografische Verlaufskontrollen bis zum 1. Lebensjahr ausreichend und werden vor der definitiven Korrekturoperation mit Stomaverschluss durch eine Miktionszysturethrografie (MCU) zum Refluxausschluss und ggf. eine MAG-3-Szintigrafie ergänzt." @default.
