- W3005130193 abstract "Latar Belakang: Ventilator-associated pneumonia (VAP) pada pasien napas dengan ventilator merupakan perhatian utama di unit perawatan intensif (ICU) karena tingginya insiden dan kematian. Dalam sistem surveilans nasional infeksi di Perancis diperoleh 12,4% dari pasien yang diintubasi 2010 terjadi VAP, dengan angka kematian 40-50%. Pneumonia dapat dicegah dengan menerapkan langkah-langkah yang tepat, karena beberapa penyelidikan dengan manajemen yang baik, VAP bisa turun sebanyak 70%. Metode: Penelitian kuantitatif menggunakan analisa observasional desain cohort. Data dikumpulkan dengan mengamati kepatuhan komponen ventilator-bundle di ICU dewasa dengan menggunakan checklist. Ventilator-bundle terdiri dari hand hygiene, oral care dengan menggunakan antiseptik, posisi tubuh pasien 300, sedation vacation dan pengkajian weaning, profilaksis ulkus peptikum, dan pencegahan deep vein thrombosis (DVT). Peneliti menggunakan CPIS (Clinical pulmonary infection score) untuk mendiagnosis pneumonia. Hasil Penelitian: Didapatkan angka kepatuhan ventilator-bundle sebesar 82,9% dan angka VAP 11,1% atau 26,7 per 1000 hari ventilator. Kepatuhan terhadap ventilator-bundle tidak memiliki hubungan dengan kejadian VAP. Dengan menggunakan odds ratio ketidakpatuhan terhadap profilaksis ulkus peptikum mempunyai risiko 8,5x terkena VAP (95% CI: 1,164-62,094). Penelitian ini membuktikan adanya faktor dari pasien yang berhubungan dengan VAP yaitu lama terpasangnya ventilator dan kejadian ETT tercabut sendiri. Pemasangan ventilator >5 hari berisiko 36 kali terkena VAP (95% CI: 3,193-405,897). Kejadian ETT tercabut sendiri berisiko 26 kali terkena VAP (95% CI: 1,796-376,303). Kesimpulan: Penelitian ini belum dapat menyimpulkan hubungan antara kepatuhan terhadap ventilator-bundle dengan kejadian VAP. Hal ini disebabkan karena jumlah sampel yang terbatas karena waktu penelitian yang singkat. Pemakaian ventilator lebih dari 5 hari, kejadian ETT tercabut sendiri, dan ketidakpatuhan profilaksis ulkus berisiko terkena VAP, sehingga harus dilakukan upaya pencegahan terhadap ketiga hal tersebut. Background: Ventilator-associated pneumonia (VAP) in ventilator assisted breathing patients is one of the main concerns in intensive care unit (ICU) because of its relation with high incidence and mortality rate. National infection surveillance in France found 12.4% of intubated patients during go infected with VAP, with mortality rate of 40-50%. Pneumonia could be prevented by implementing the right precaution, investigations show that with good management, VAP could be reduced until 70%. Methods: Quantitative study using observational analysis with cohort design. Data collected by observing compliance of ventilator-bundle in adult ICU with checklist. Implementation of ventilator-bundle could be observed directly and by checking patients hemodynamic record. Ventilator- bundle consist of hand hygiene, oral care with antiseptic, patients body position at 30° angle, sedation vacation, and weaning assessment, peptic ulcer prophylaxis, and prevention of deep vein thrombosis (DVT). This study use CPIS (clinical pulmonary infection score) to diagnose pneumonia. Results: Ventilator-bundle compliance rate are recorded on 82.9% and VAP on 11.1%, or 26.7 per 1000 ventilator days. Compliance with the ventilator bundle has no relationship with the incidence of VAP. By using the odds ratio of non-compliance with peptic ulcer prophylaxis has 8,5x risk of VAP (95% CI: 1.164 to 62.094). This study proves the existence of the patients factors associated with VAP which are duration of ventilation and incidence of spontaneous ETT pulling. Installation of ventilator for more than 5 days has 36 times greater risk of VAP (95% CI: 3.193 to 405.897). Spontaneous ETT pulling has 26 times greater risk of VAP (95% CI: 1.796 to 376.303). Conclusion: This study couldnt summarize the relation between ventilator-bundle compliance with VAP incidence. This result caused by the limited subjects involves.
