We agree entirely that patients with breast cancer should be managed by experts, discussing available evidence in a multidisciplinary setting. This enables selection of a range of surgical options for patients, and clearly eOPBCS should be offered only to women with appropriate tumours by surgeons with appropriate skills. Your correspondence suggest that these decisions should be based on ‘updated scientific knowledge and surgical experience adapted to specific patient value’, and rightly point out the lack of any trial data in this field.
