Lung development is regulated by multiple factors and includes three sequential stages: the pseudoglandular, the canalicular, and the saccular. The lung is divided anatomically into conducting, transitional, and respiratory zones (the latter two constituting the parenchyma). The conducting zone consists of bronchi and non-alveolated bronchioles. The transitional airways (respiratory bronchioles [RBs] and alveolar ducts [ADs]) both conduct gas and participate in respiration. The parenchyma can be subdivided into the primary lobule, the secondary lobule, and the acinus. The acinus can be considered a functional unit for gas exchange, and comprises lung tissue distal to a terminal bronchiole, including RBs, ADs, alveolar sacs, and alveoli. The airway epithelial cells are mostly ciliated columnar or cuboidal cells; scattered mucus-secreting (goblet) cells are also present. The alveoli are lined by type I and II epithelial cells. The alveolar interstitium contains reticulin, elastic fibres, and collagen, which interconnect with connective tissue in the airways and vessels, providing a complex, three-dimensional ‘skeletal’ framework. The lungs receive a dual blood supply from the pulmonary and bronchial arteries and are innervated by the vagus and sympathetic nervous systems.
