- W3007988880 abstract "To assess the feasibility of single puncture double sheath insertion method in an unilateral femoral vein (two-in-one method) for adrenal venous sampling (AVS). Two 5-Fr sheaths were percutaneously inserted into a single femoral vein for AVS before and after adrenocorticotropic hormone stimulation including segmental branch sampling in 324 patients. Two needles were inserted in tandem under ultrasound guidance; furthermore, two sheaths were subsequently inserted if there were two punctures. In case of a single puncture, two sheaths were inserted through two guidewires, which were inserted through a temporarily inserted sheath (two-in-one method). This protocol was repeated until two sheaths were successfully inserted and the following parameters were evaluated: technical success rates, complications, and procedure time. Two sheaths were inserted with the two-in-one method in 56 (17.3%) patients. The technical success of catheterization was noted for the right and left adrenal veins in 53 (94.6%) and 56 patients (100%), respectively. The technical success rates of segmental branch sampling of at least two effluent tributaries for the right and left adrenal veins were 80.4% and 100%, respectively. Minor groin hematoma was observed in 5 patients (8.9%). No major groin hematoma, deep venous thrombosis, or pulmonary embolization was observed. Mean number of needle punctures was 2.9 ± 1.7 and time for double sheath insertion was 7.8 ± 5.5 minutes. Single puncture double sheath insertion method in an unilateral femoral vein is feasible and would reduce the procedural time and patient discomfort for AVS." @default.
