- W3008049301 abstract "As the technology has arrived to test newborns' urine or saliva samples by polymerase chain reaction (PCR) for congenital infection due to cytomegalovirus (cCMV), the “who” of screening is as yet unsettled. The “why” of screening would be to detect infants with extant or risk to develop hearing loss as a sequelae of cCMV infection. Should screening be universal, ie, to include all healthy newborns and all potential infections? Or should it be targeted, eg, to infants who fail the newborn screening test for hearing (NBHS), which would miss some infected infants who could lose hearing over time? With the additional lack of understanding of the “what to do” with a positive PCR test result, or knowledge of potential unintended consequences of labeling for parents their infants as cCMV-infected, 11 state legislatures (starting with Utah in 2013) have enacted laws to mandate targeted screening for cCMV of infants failing NBHS, or educational programs about cCMV, or both. Diener et al utilized a detailed questionnaire offered to parents in the waiting area of a pediatric ENT clinical in a tertiary care children's healthcare facility in Salt Lake City in 2016 and 2017. They sought to assess the 365 parents' (92% of whom were mothers) knowledge of CMV, its transmission, and effects of cCMV as well as attitudes toward screening, to include concerns about stigmatization, un-needed doctor visits and cost, and willingness to pay $20 to $50 for screening. Using answers to questionnaires, investigators categorized parents into clusters according to knowledge and attitudes (Figure), and then analyzed associations/interactions of clusters. They found that knowledge of CMV, cCMV, and the Utah law were low. Attitude toward cCMV screening generally was positive. The majority of mothers would want to know if their child had cCMV even if asymptomatic. Most were not deterred by potentially negative consequences and were willing to pay $20 to test. Caregivers in the “highly knowledgeable” cluster were more likely to strongly favor cCMV screening. Relative to cCMV screening, the authors suggest necessary next steps to enhance provider knowledge and patient-provider communication.
