- W3008168880 abstract "To develop an open source toolkit to simulate and aid pre procedure planning of percutaneous microwave ablation and cryoablation procedures using the Python scripting language and Slicer3D, an open-source medical imaging platform with advanced three-dimensional graphic processing and segmentation functionality. An extension of Slicer3D was chosen for implementation of the toolkit. The microwave ablation module is modeled after PR XT probe (NeuWave Medical, Madison, WI). Burn pattern can be adjusted as a function of time based on NeuWave specifications. The cryoablation module is modeled after PCS-17R probe (EndoCare, Westminster, London). Ablation zone is fixed based on EndoCare specifications. The toolkit allows user specification of number and geometric positioning of ablation probes overlaid on patient MRI or CT data. The user may specify a margin around the tumor border to calculate evaluation metrics. The simulated ablation zones are displayed in axial, coronal, sagittal views, and 3D rendering. Evaluation metrics (% of tumor/organ ablated) are calculated and displayed to the user. For evaluation of the toolkit, two anonymized patient cases with MRI imaging were selected: hepatocellular carcinoma using microwave ablation and renal cell carcinoma using cryoablation. The native segmentation workflow in Slicer3D was employed to segment relevant structures in each case, including liver/kidney, tumor, bone, air, and vasculature. The planning toolkit yielded 100% coverage of tumor with 0.5 cm margin for both case studies. The toolkit is able to detect violation of any anatomic constraints, such as intersections of the ablation zone with bone, blood vessels, or lung tissue. The toolkit allows the user to specify a range of potential probe insertion angles and calculates the optimal configuration using an internal scoring system that maximizes the percentage of tumor ablated while penalizing violations of anatomic constraints. An open source planning and simulation toolkit was successfully built as a module to Slicer3D. This toolkit may augment preprocedural planning of ablation procedures by interventional oncologists and reduce the rate of tumor recurrence." @default.
