Previous versions: March 4, 2020: Ahead of Print In Response:We read with interest and thank Dr Jean Fiehler and his colleagues for their comments on our article “Impact of Reperfusion for Nonagenarians Treated by Mechanical Thrombectomy: Insights From the ETIS Registry.”1 We agree on the main message of our study, that is, nonagenarians should not be excluded systematically from mechanical thrombectomy. Our data suggest an important role for a complete recanalization, even among nonagenarian stroke patients, although our limited population does not allow us to definitively conclude on the role of first-pass effect. Of course, the objective of the endovascular procedure is to achieve a complete recanalization, even if it occurs after several passages of devices. We fully agree on the value of working on the Thrombolysis in Cerebral Infarction (TICI)-RANKIN mismatch to identify associated factors and develop neurorehabilitation strategies.EveDrouard-de Rousiers, MDDepartment of Neurology, Stroke Center, Foch Hospital, Suresnes, FranceLudovic Lucas, MDIgor Sibon, MD, PhDDepartment of Neurology, Stroke Unit, Bordeaux University Hospital, FranceBertrand Lapergue, MD, PhDDepartment of Neurology, Stroke Center, Foch Hospital, Suresnes, FranceDisclosuresDr Lapergue reported research grants from Stryker, Penumbra, and Microvention.
