- W3021162841 abstract "To examine the incidence of cognitive impairment within general medical patients and to examine the relationship between impairment and behavior on the ward. Scores on the Abbreviated Mental Test (AMT) were routinely collected from consecutive admissions in general medical wards in two large metropolitan hospitals in Melbourne Australia. Where a score of seven or lower was obtained, the patient's behavior was observed by nursing staff for at least seven days and recorded on the Cohen–Mansfield Agitation Inventory (CMAI). In total, data on 683 patients were collected in the study. About two thirds of the patients in the sample were aged 60 years and over and 56 percent were female. Overall 18.3% of patients scored seven or lower on the AMT and were considered to be cognitively impaired but low scores were strongly age related with 43% of the 85 years and older age group scoring seven or lower. The majority of patients (65.6%) in the impaired category exhibited at least one instance of agitated behavior in the seven days following admission. Logistic regression analysis confirmed a significant relationship between the presence of agitated behavior and the recorded AMT, i.e., patients with a low AMT score were more likely to exhibit agitated behavior. There was no significant relationship between age or gender and agitated behavior after AMT score was taken into account. However setting (Hospital A or Hospital B) was also an independent predictor of agitated behavior with the proportion of patients with ATM scores of seven or less who displayed agitated behavior varying considerably between the two locations (from 44.4% to 85.7%) A relative frequency of agitated behaviors in the sample was obtained by a consideration of the frequency of different behaviors across all patient observation days (i.e. number of patients observed by days observed). This analysis indicated that ‘general restlessness’ was the most frequent agitated behavior recorded followed by ‘repetitive sentences/questions’ and ‘constant requests for attention.’ Findings are discussed in terms of the age trends in hospital inpatient populations, the implications for patient behavior and the possible adaptations required of hospital procedures and practices." @default.
