- W3021360151 abstract "The beneficial effects of estrogen to women's overall health are often overshadowed by its negative effects in the breast and uterus; hence, attempts have been made to develop drugs that selectively block estrogen's negative effects while maximizing all of its positive attributes. These attempts have resulted in the development of selective estrogen receptor modulators (SERMs), which are compounds that potentially exhibit estrogen agonist activity in some tissues (i.e., bone, heart, central nervous system) and estrogen antagonist activity in other tissues (breast and uterus). Initially, SERMs were considered antiestrogens but it was subsequently recognized that this categorization inadequately described their spectrum of activities. Tamoxifen (Nolvadex), the most well-known of the SERMs, is indicated for the treatment and prevention of estrogen receptor (ER)-positive breast cancer, whereas, raloxifene (Evista), another SERM, is approved for the treatment and prevention of postmenopausal osteoporosis. It should be noted that raloxifene (then named keoxifene) was originally developed as a breast cancer drug by Eli Lilly and Company; however, it was discontinued after it was found to be ineffective as a second-line treatment following the failure of tamoxifen. The findings that raloxifene protected ovariectomized rats from bone loss and prevented rat mammary carcinogenesis resulted in the eventual development of raloxifene for the treatment and prevention of osteoporosis with breast and endometrial safety. This review traces the development of raloxifene from its failure as a breast cancer drug to its reinvention as an antiresorptive medication and a breast cancer preventive." @default.
