The major goals of treatment are to safeguard the mother from acute dangers or irreversible insults during or immediately after the pregnancy while delivering a healthy infant. In this respect, one balances short-term maternal outcome against possible long-term consequences of intrauterine drug exposure or hemodynamic insult on fetal and childhood growth and development. While comorbidities such as diabetes mellitus, target organ dysfunction at baseline, and secondary causes of hypertension may certainly interact with maternal hypertension and alter strategies for its control, these concerns will not apply to most hypertensive gravidas. The majority of women with blood pressures high enough to warrant treatment during pregnancy will either have chronic essential hypertension or preeclampsia leading to threatening levels of pressure which occur when there are compelling reasons to extend the length of the pregnancy. It should be noted that the balance of risks and benefits for antihypertensive therapy will differ for women with chronic hypertension present from early in gestation, whose fetuses may have greater drug exposure during early stages of development, compared with women who develop hypertension closer to term. Well-designed and adequately powered trials, of which there are shamefully few and too many poorly designed, are still needed to account for this clinical heterogeneity. Maternal risks which may justify pharmacotherapy include that of superimposed preeclampsia which, with its morbid outcomes, appears to account for most complications ascribed to chronic hypertension." @default.
