- W3026536315 abstract "Although the 6-item Kessler psychological distress scale (K6) is widely used as a reliable screener tool for non-specific psychological distress (PD), the connotation of items used to assess PD may vary across cultures and populations, particularly among marginalized groups. This study aimed to evaluate the psychometric properties of the K6 scale among women who exchange sex (WWES). Data were obtained from WWES (n=333) who participated in the National HIV Behavioral Surveillance of high-risk heterosexual women in Houston, Texas in 2016. PD was measured using the K6 scale designed to assess nonspecific PD in the past 30 days using a Likert scale that ranged from 0 to 4. We applied the Item Response Theory model and Confirmatory Factor Analysis to examine the dimensionality of the K6 scale and detect the differential item functioning (DIF) in WWES. The K6 items correlations were robust (0.29-0.64) and internal consistency reliability (Std α = 0.77) suggests that the items continuum provided adequate measurement of the latent construct. However, the amount of information provided by each item as determined by the slope (discrimination) parameter estimates ranged from 0.50 (Item 5) to 3.74 (Item 6) [All slopes: p<0.0001] and were uneven across the entire continuum of the latent construct. The items threshold (difficulty) parameters ranged from -0.76 (Item 6) to 1.90 (Item 5) [All thresholds: p<0.0001], with 54% of the values being <0, suggesting the response options are relatively easy and therefore may be useful in discriminating among WWES having lower abilities. The K6 scale provided uneven information across the continuum of the latent construct, thus, indicating DIF among WWES. The measurement invariance suggests the need for refinement of the scale to include items that can provide adequate precision across the range of the latent construct continuum and enhance more robustness." @default.
