- W3028366909 abstract "To examine the interpretability and statistical validity of the Life Expectancy Ratio (LER) as an alternative effect measure to the Hazard Ratio (HR) in indirect comparisons of time-to-event data. The LER between two treatments is defined as the ratio between the areas under the modelled survival curves of two treatments. As such, LER is an alternative to the HR under proportional hazards model, and a meaningful ratio-based effect measure under non-proportional hazards where the HR is not. We examined interpretability and statistical validity of LER versus HR in a indirect comparison of dacomitinib vs afatinib for patients with locally advanced Non-small cell lung cancer. The overall survival (OS) data did not meet the proportional hazards assumption, whereas the progression-free survival (PFS) data did. Second-order fractional polynomials models were fit to OS and PFS. We extracted the estimates of LER and HR at time points: 12, 24, 36 and 48 months. For OS where a non-proportional hazards model was appropriate, the LER was stable over time points, starting at 0.96 (95%CrI; 0.88-1.01) at 12 months and ending at 0.92 (95%CrI; 0.77-1.08) at the 48 months timepoint. Although not a meaningful measure, the extracted HR based on non-proportional hazards evolved as 0.77, 0.84, 2.85 and 23.6 for the 12, 24, 36, and 48 months timepoints. For PFS where the hazards were proportional, the LER was also stable starting at 1.00 (95%CrI; 0.98-1.02) with the first 12 months of data and ending at 1.02 (95%CrI; 0.88-1.17) at 48 months of data. This is compared to the proportional hazards based HR of 0.97 (95%CrI; 0.65-1.40). The Life Expectancy Ratio is an intuitive effect measure with a similar interpretation to the hazard ratio. It is meaningful for non-proportional hazard and comparable for proportional hazards settings." @default.
