- W3028630527 abstract "ObjectiveTo evaluate what effect can result from negative fluid balance in the first 72 hours of mechanical ventilation initiation in acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients in ICU.Methods43 patients with AECOPD needed mechanical ventilation who admitted to the intensive care unit of the 901th Hospital of the Joint Logistics Support Force of the Chinese People′s Liberation Army (formerly PLA 105th Hospital) from January 2016 to December 2017 were randomized into two groups, 22 of fluid administration group received therapeutic target of negative fluid balance>2 500 ml in the first 72 hours, the rest of control group received therapeutic target of fluid balance.Then the heart rate (HR), mean artery pressure (MAP), centre venous pressure (CVP), platform pressure (Pplat), oxygen partical pressure fraction of inspired oxygen (PaO2/FiO2), pH, partial pressure of carbon dioxide (PaCO2), lactic acid (LAC), brain Natriuretic peptide (BNP), duration of ventilation, ICU stay and 28-day mortality would be measured and analyzed.ResultsAfter 72 hours treatment, there showed significant difference in each group in many important parameter compared with that before treatment (P<0.05). It is improved significantly in CVP(t=-3.27, P=0.002)、Pplat (t=-2.70, P=0.01), PaO2/FiO2(t=2.49, P=0.02) and BNP (t=-2.29, P=0.03) in fluid administration group compared with control group.So was the duration of ventilation、ICU stay between the two groups.But Compared with control group, there showed no significant difference in 28-day mortality in fluid administration group.ConclusionsTo AECOPD patients with mechanical ventilation, the negative fluid balance is beneficial to the improvement of oxygenation, reduction of duration of ventilation and ICU stay.Key words: Ventilators, mechanical; Pulmonary disease, chronic obstructive; APACHE; Fluid management" @default.
