- W3028986952 abstract "Objective To investigate the effect of ulinastatin (uli) administered intraoperatively on preventive analgesia.Methods 80 elderly patients undergoing lower extremity orthopedic surgery were randomly divided into four groups.Patients in group A received uli 2×105 IU before skin incision,then 2×105 IU/h continuous infusion until the end of the operation; Patients in group B were given 1% ropivacaine 5 ml-10 ml into epidural space 15 min-20 min preoperatively and additional 5 ml-10 ml every 45 min-60 min during the operation; combined protocol of group A and group B was given to patients in group C.Group D was control group.Patient's sex,age,weight,height,ASA classification,operation type,operation duration,blood loss,the first time of pressing the PCEA,and the frequency of effective or actual press on the PCEA were recorded.The visual analog scale (VAS) was obtained and blood samples were collected for measurement of IL-6,IL-10.Results Sex,age,weight,height,ASA classification,operation style,operation duration,blood loss and the level of IL-6,IL-10 preoperatively were similar among the four groups.After surgery,the concentration of IL-6 decreased in group A and C,increased in group B and D,while that of IL-10 increased in group A and C,decreased in group D,and merely changed in group B.Preoperative VAS scores showed no significant difference among each groups.After surgery,all groups achieved satisfactory at rest,but the score of group D was higher than that of the other groups at any day; while with movement,all groups achieved satisfaetory except group D.The VAS score of group C decreased on the 1st day after the operation,while that of group D was still above 3 on the 7th day.Conclusion Regulating the systemic inflammatory response with ulinastatin intraoperatively can relieve postoperative pain to some extent,but is not as effective as epidural anesthesia which has been confirmed to show a preemptive analgesia effect in previous studies.The combination of the above two methods may provide a better outcome.Key words: Ulinastatin; Systemic inflammatory response syndrome; Preemptive analgesia; Interleukin" @default.
