- W3028994097 abstract "Objective To investigate the diagnostic value of Mycobacwrium tuberculosis specific cellular immune responses assays and antibodies detection. Methods Mycobacterium tuberculosis specific antibodies were detected by using Immune colloidal gold technique, while the specific cellular immune responses were determined by measuring the ESAT-6 specific IFN-γ releases using enzyme-linked immunospot assay(EIJSPOT). A total of 90 healthy controls and 113 patients with tuberculosis were recruited for these comparative assays. Among them, the 90 healthy controls were also received PPD skin test. The performance of ELISPOT and commercial T-SPOT. TB were compared in parallel in 12 patients with pulmonary tuberculosis and 10 healthy controls. Results The cut-off value of ELISPOT assay was set as 30 spots/2×105 cells based on receiver operating characteristic (ROC) curve analysis, with which the sensitivity and specificity were 75.2% and 85.6%. Among the healthy controls, the positive rate of antibody test and ELISPOT were 26.67% and 14.44%. Of them, 25 (27.78%) showed strong response to the PPD skin test. Both the results of antibody test and ELISPOT were significantly correlated with the PPD skin test (r=0.280, P=0.008; r=0.253, P=0.018). Among the 113 patients with tuberculosis, 77.88% were positive for the ELISPOT assay and 60.18% were positive for antibody test. The positive and negative predictive value of ELISPOT were 86.27% and 75.25%, respectively; both of which are significantly higher than those of antibody test (73.91% and 59.46%, respectively). The comparative study indicated that there was no statistical difference between our in-house ELISPOT and T-SPOT. TB assay (χ2=0.500, P=0.480). Conclusions Compared to the antibody test, mycobacterium tuberculosis ESAT-6 specific IFN-γ ELISPOT has higher specificity, sensitivity, the positive and negative predicative value.
