- W3029058291 abstract "To the Editor: Foreign body ingestion continues to be a frequent problem faced by pediatric gastroenterologists(1). The most recent guidelines recommend abdominal x-rays with emphasis on a 2-view approach (2,3). The guidelines are unclear as to whether upright or supine radiographic evaluation is recommended. We would like to inform the readers of 2 cases wherein the location of the foreign body appeared to be distal to the stomach on upright films. Subsequent supine x-rays suggested an intragastric location that was confirmed by endoscopy. 1. Upright and supine AP views were obtained on a 10-year-old boy (Fig. 1A, B). The upright view suggested a position distal to the stomach. Subsequent supine x-rays showed the ingested objects in the left upper quadrant, most likely in the stomach (not shown). The patient underwent upper endoscopy where 2-disc magnets were discovered in the area of the gastric fundus. 2. Upright AP abdominal and lateral x-rays of a 5-year-old boy revealed a foreign body projecting over the mid-abdomen (not shown). Repeat imaging showed an apparent shift of the object to the left upper quadrant of the abdomen in the supine position (Fig. 1C, D). The patient underwent upper endoscopy where an intragastric marble was successfully retrieved. FIGURE 1: A) Patient 1 - XR Abdomen AP (upright). B) Patient 1 - XR Abdomen AP (supine). Proper localization is particularly important when considering observation versus endoscopic intervention (2,3).
