- W3029483046 abstract "ObjectiveTo investigate the effects of serum inflammatory factor levels on cardiac function and incidence of atrial fibrillation in elderly patients with heart failure and pulmonary infection.MethodsA total of 124 elderly patients with heart failure complicated by pulmonary infection treated in Shanxi General Hospital from August 2015 to August 2017 were selected as observation group, and 120 elderly heart failure patients without pulmonary infection treated at the same period were selected as control group. The levels of brain natriuretic peptide (BNP), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and procalcitonin (PCT) were compared between the two groups. The white blood cell count (WBC), neutrophil percentage (NEU%) and lymphocyte percentage (LYM%) were compared between the two groups. And the incidences of atrial fibrillation of the two groups were recorded.ResultsThe BNP, TNF-α, IL-6 and PCT of observation group were (1 187.72±571.63) ng/L, (18.35±3.62) ng/L, (35.21±7.32) ng/L, (15.95±8.03) μg/L, respectively, which were higher than those in control group, which were (927.03±354.91) ng/L, (12.04±3.51) ng/L, (23.63±6.26) ng/L, (3.49±1.38)μg/L, respectively. With the increase in NYHA grade of myocardial function, the level of above inflammatory factors also increased, and the difference was statistically significant (P<0.05). The WBC, NEU% and LYM% of observation group were (15.84±0.51)×109/L, (82.74±20.35)% and (43.56±11.47)%, respectively, which were significantly higher than those in control group, which were (8.37±0.24)×109/L, (74.32±12.8)%, (31.26±12.24)%, respectively (P<0.05). Pearson analysis showed that there was a positive correlation between BNP, TNF-α, IL-6, PCT and cardiac function grade (r=0.189, 0.793, 0.914, 0.616, all P<0.05). According to the 6-months follow-up, the incidence of atrial fibrillation in observation group was 10.74%. The levels of serum inflammatory factors and BNP in patients with atrial fibrillation were higher than those in non-atrial fibrillation patients, and the difference was statistically significant (P<0.05).ConciusionsThe levels of serum inflammatory factors and BNP are elevated in patients with heart failure complicated by pulmonary infection, resulting in aggravation of cardiac function damage in elderly patients. And the expression of inflammatory factors is related to the incidence of atrial fibrillation.
