- W3030489505 abstract "ObjectiveTo analyze the clinical characteristics and prognosis of hepatoblastoma (HB) children older than 3 years.MethodsOne hundred and sixty-one cases of HB were collected from November 2006 to March 2014 in Department of Pediatrics, Beijing Tongren Hospital, Capital Medical University.Twenty-nine of them were older than 3 years at confirmation diagnosis.According to Children Oncology Group after operation, the clinical stages of HB were from Ⅰ stage to Ⅳ stage.Treatment methods mainly included pre-operation chemotherapy, operation and post-operation chemotherapy.Solitary lesion of HB could be treated with interventional chemotherapy and operation.Resistant cases of HB could be treated with high dose of chemotherapy and autologous peripheral blood stem cell transplantation.Analysis of the characteristics of the disease, clinical efficacy and prognosis were performed by using SPSS 17.0 software.Results(1) In 29 cases, 17 cases were male, 12 cases were female.The medium age was 4.58 years old (3 years and 1 month-15 years old). Twenty-four cases were >3-6 years old(accounting for 82.8%), 5 cases were older than 6 years old(accounting for 17.2%). Thirteen cases were in Ⅲ stage (44.8%), 16 cases were in Ⅳ stage(55.2%). (2) All of the patients got follow-up.The medium time of follow-up was 18.5 months up to January 2016, during which 9 cases died (4 cases gave up, 5 cases died of intracranial metastasis), survival was in 20 cases, and overall survival rate (OS) was 68.9%.The event free survival was 11 cases, accounting for 37.9%.According to Kaplan-Meier survival analysis, OS of 1 year, 2 years, 3 years was 93.5%, 83.4%, and 78.2% respectively.(3) In the cases older than 6 years: 3 cases of Ⅳ stage died of recurrence after treatment, while 2 cases of Ⅲ stage had complete remission.In 24 cases of >3-6 years old: 4 cases gave up, 2 cases died, 18 cases survived, and the OS was 90.0%(18/20 cases).ConclusionsThe clinical stage is late and the prognosis is poor in older children of HB.Therefore, older patients of HB are sensitive to chemotherapy, and are mainly treated through operation and chemotherapy.Key words: Hepatoblastoma; Older child; Chemotherapy; Prognosis; Clinical efficacy" @default.
