- W3030658889 abstract "目的肺动脉高压(pulmonary hypertension,PH)患儿疾病负担及病死率高于PICU内普通患儿。本文旨在建立一项用于预测PICU内PH患儿病死率的风险校正工具:儿童肺动脉高压重症监护病死率指数(pediatric index of pulmonary hypertension intensive care mortality,PIPHM)评分。设计回顾性分析前瞻性收集的多中心儿童重症监护数据。场所 143家于2009年1月1日至2015年12月31日向虚拟儿科系统数据库传递数据的医疗中心。对象21岁及以下的PH患者干预措施 于患者入PICU 12 h内采集21项人口、诊断及生理学数据以评估是否入组。采用多参数Logistic回归及逐步回归法建立最终模型。采用受试者工作特征(receiver operating characteristic,ROC)曲线比较PIPHM评分、儿童病死率风险-3(pediatric risk of mortality-3,PRISM-3)评分及儿童病死率指数-2 (pediatric index of mortality-2,PIM-2)评分。测量方法及主要结果共纳入14 268例入院诊断为PH的患儿,主要预后指标为PICU病死率。筛选14项数据建立最终模型:年龄、心动过缓、低收缩压、呼吸急促、pH、FiO2、血红蛋白、血尿素氮、肌酐、机械通气、非选择性入院、既往曾入PICU、因非手术治疗的心血管疾病入PICU、入PICU前短时间内心跳骤停。PIPHM模型(AUC=0.77)ROC曲线明显优于PRISM-3(AUC=0.71,P<0.001)及PIM-2(AUC=0.69,P<0.001)。结论PIPHM评分为一项精简模型,用于预测PICU内PH患儿多中心队列病死率时优于PRISM-3及PIM-2。将PIPHM模型用于PICU内PH患儿或有助于早期确认高死亡风险患儿并提高为患儿及家属提供预测的能力。" @default.
