- W3031775235 abstract "Objective To investigate the safety and efficacy of late steroids withdrawal in Chinese stable kidney transplant (KTx) recipients.Method Twenty-four KTx patients with stable graft function who were intolerant to side effects of steroids were enrolled.Steroids were withdrawn in stepwise manner after increasing mycophenolate mofetil (MMF) dose to 1.25-1.50 g/day and stabilizing efficient blood concentration of calcineurin inhibitors (CNIs).Results The median followup time was 18.9 months after complete steroids withdrawal.All allografts survived.Two incidents of acute rejection occurred in one patient (4.2％) due to insufficient MMF dose.Renal allograft function was decreased in one patient due to CNI nephrotoxicity.After steroids withdrawal,84.2％ (16/19) diabetic or hyperglycemic patients recovered to normal fast blood glucose,one of whom was subjected to the simultaneous pancreas-kidney transplant recipient.Five obesity patients maintained well-controlled body weights,and clinical symptom was alleviated in patients with blurred vision (n =2) or with osteonecrosis of the femoral head (n =1).Steroids withdrawal significantly decreased fast blood glucose from 6.54 ± 1.84 mmol/L to 5.48 ± 1.14 mmol/L (P =0.02),and decreased triglycerides from 2.31 ± 1.20 mmol/L to 1.27 ± 0.56 mmol/L (P =0.0038).Conclusion Late steroids withdrawal is feasible in stable kidney transplant recipients to decrease its side effects.It is important to provide sufficient MMF dose and stable CNIs exposure.Steroids withdrawal significantly decreased hyperglycemia and hyperlipidemia,and thus may decrease cardiovascular diseases.Key words: Kidney transplantation; Steroids" @default.
