- W3032283252 abstract "ObjectiveTo study the clinical effect of fiberoptic bronchoscopy and anti-infection therapy on severe ventilator associated pneumonia (VAP) and its influence on respiratory mechanics indexes, and to provide reference for the clinical treatment of severe ventilator associated pneumonia.MethodsSeventy-five cases of severe VAP from February 2012 to June 2016 were selected, according to the different treatment methods, they were divided into observation group (n=41) and control group (n=34). The two groups were given conventional antibiotic therapy, the observation group was treated by fiberoptic bronchoscopy lavage, and the control group was treated by conventional infusion. The total effective rate, respiratory mechanics indexes level, WBC recovery time, mechanical ventilation time, cooling time, ICU before hospitalization time and treatment, CPIs of three, five and seven days after treatment were compared.ResultsThe total effective rate of observation group was 95.12% (39/41), and the total effective rate of control group was 76.47% (26/34), the difference was significant (P<0.05); one and four hours after treatment, the Cdyn indicators were higher than before treatment in both groups, the WOB, PIP, Raw index levels were lower than those before treatment, the differences were significant (P< 0.05); one and four hours after treatment, the Cdyn indicators of observation group were higher than those of control group, the WOB, PIP, Raw index levels were lower than those of control group, the differences were significant (P<0.05); In observation group, the WBC recovery time, mechanical ventilation time, cooling time, ICU hospitalization time were lower than those in the control group, the differences were significant (P<0.05). Three, five and seven days after treatment, the CPIS of observation group was lower than that of control group, the difference was significant (P<0.05).ConclusionsFiberoptic bronchoscopy assisted anti-infection therapy for severe VAP, can accelerate the recovery of the patients body, effectively control the infection, improve the respiratory mechanics index level, with high practical value.Key words: Anti infection; Respiratory mechanics index; Fiberoptic bronchoscopy; Severe ventilator associated pneumonia" @default.
