- W3034778852 abstract "To investigate the effectiveness of autogenous tendon reconstruction under total arthroscopy in the treatment of chronic Achilles tendon rupture.Between June 2015 and June 2018, 16 patients with chronic Achilles tendon ruptures were treated by autogenous tendon reconstruction under total arthroscopy. Of the 16 patients, 11 were males and 5 were females. Their mean age was 40.7 years (range, 21-55 years). The disease duration was 14-20 months (mean, 16.4 months). Preoperative American Orthopaedic Foot and Ankle Society (AOFAS) score was 41.2±2.2 and the pain visual analogue scale (VAS) score was 7.9±1.2. MRI and B-ultrasonography examinations showed that the Achilles tendon was not continuous. The length of Achilles tendon defect was 5.0-10.3 cm, with an average of 5.8 cm. The rupture of the Achilles tendon happened on top of the insertion of the tendon in 4 cases and at the tendon-muscle belly connection in 12 cases. The operation time, intraoperative blood loss, hospital stay, and related complications were recorded. The AOFAS score and VAS score were used to evaluate the improvement of ankle joint function and pain.The average operation time was 77.2 minutes (range, 60-90 minutes). The average intraoperative blood loss was 20.5 mL (range, 15-30 mL). The average hospital stay was 7.2 days (range, 5-10 days). All incisions healed by first intention. There was no skin necrosis, infection, or deep vein thrombosis. All the patients were followed up 8-18 months, with an average of 12 months; and 10 cases were followed up more than 12 months. During the follow-up, there was no Achilles tendon re-rupture, and the symptoms of pain and heel lifting failure significantly improved. MRI reexamination showed that the continuity of Achilles tendon recovered. At 1, 3, 6, and 12 months postoperatively, AOFAS scores significantly improved and VAS scores significantly reduced, except for 1 month postoperatively, the scores at other time points were superior to that before operation, the differences were significant ( P<0.05).Autogenous tendon reconstruction under total arthroscopy in the treatment of chronic Achilles tendon rupture has the advantages of small trauma, rapid functional recovery, and satisfactory surgical efficacy.探讨全关节镜下自体肌腱重建治疗陈旧性跟腱断裂的疗效。.2015 年 6 月—2018 年 6 月,采用全关节镜下自体肌腱重建治疗 16 例陈旧性跟腱断裂。男 11 例,女 5 例;年龄 21~55 岁,平均 40.7 岁。病程 14~20 个月,平均 16.4 个月。美国矫形足踝协会(AOFAS)评分为(41.2±2.2)分,疼痛视觉模拟评分(VAS)为(7.9±1.2)分。MRI 以及 B 超检查提示跟腱连续性不佳。跟腱缺损长度为 5.0~10.3 cm,平均 5.8 cm。4 例于跟腱止点上方断裂,12 例为腱腹交界区断裂。记录手术时间、术中出血量、住院时间及相关并发症发生情况,采用 AOFAS 评分及 VAS 评分评价关节功能及疼痛改善情况。.手术时间 60~90 min,平均 77.2 min。术中出血量 15~30 mL,平均 20.5 mL。术后住院时间 5~10 d,平均 7.2 d。术后切口均Ⅰ期愈合,无皮肤坏死、感染以及下肢深静脉血栓形成等并发症发生。患者均获随访,随访时间 8~18 个月,平均 12 个月;其中 10 例随访达 12 个月以上。随访期间均未出现跟腱再断裂,疼痛以及提踵不能症状均较术前明显改善。MRI 复查提示跟腱连续性恢复。术后 1、3、6、12 个月 AOFAS 评分均较术前明显提高,VAS 评分明显降低,除术后 1 个月外,其余时间点与术前比较差异均有统计学意义( P<0.05)。.全关节镜下自体肌腱重建治疗陈旧性跟腱断裂,具有创伤小、术后功能恢复快的优势,可获得较好疗效。." @default.
