Spiritual care is recognized as an integral component of cancer care, but little is known about the spiritual care needs or the utilization of chaplaincy services among hospitalized patients in the US who identify with the Eastern faiths (Hinduism, Buddhism, Sikhism). The overall objectives of this study are to describe utilization of chaplaincy services and spiritual care needs of patients of Eastern faiths hospitalized at a dedicated cancer center and to explore how these factors may be associated with end-of-life (EOL) healthcare utilization. This is a retrospective review of all adult (≥18 years) cancer patients admitted to Memorial Hospital between 2015-2019 who identified as Hindu, Buddhist, or Sikh. Chaplaincy utilization was determined from the number of referrals and documentation in the EHR. Spiritual needs were identified from chaplaincy and Supportive Care Service (SCS) consultation documentation based on NCCN guidelines for chaplaincy assessment. SCS documentation was also reviewed for patient’s self-reported spiritual distress (routinely assessed with other symptoms). Hospital length of stay (LOS), chemotherapy within last 2 weeks of life, ICU care within last 30 days of life, and hospice enrollment within last 6 months of life were determined from hospital administrative databases. We identified 2351 admissions of 1108 patients including 671 (61%) Hindus, 383 (34%) Buddhists, 54 (5%) Sikhs, with median LOS 4 days. For these admissions, chaplains documented 631 (27%) initial visits to patients, of whom 613 were available. The most common spiritual need was grief/loss. Services requested from chaplaincy included prayer and identifying resources from an individual’s faith community. SCS was consulted on 233 (10%) admissions. Assistance with grief/loss is among needs identified by chaplaincy and supportive care specialists for spiritual care of hospitalized cancer patients of Eastern faiths. Deeper understanding of these needs will support care that is sensitive to this group." @default.
