- W3036697984 abstract "After gastroesophageal reflux swallowed saliva should arrive to the distal esophagus. Salivas bicarbonate is critical to neutralize the acidified d~stal esophageal mucosa. It has been suggested that abnorntal esophageal motility might affect saliva transport inducing a delayed acid neutrahzation. We aimed to evaluate the role of esophageal penstalsis and on saliva transport to the distal esophagus Methods: Esophageal transit of 2 ml of artificial saliva and esophageal motility were studied concurrently in 7 asymptotnatic volunteers (4 men) bdbre and after disruption of motility m the smooth muscle portion of the esophageal body with sildenatll (50 mg). ?d}er 2 swallows to empty the oral cavity from endogenous saliva, artificial sahva (2mL)(pH:6 9,Na=41mmol/l K= 19mmoH, CO3H= 3.8 mMogl) labeled with 18.5 MBq of 99Tc was administered in the mouth Each subject performed 2 swallows of artificial sahva in upright and 2 swallows in supine position and saliva transit was measured scintigraphioaly during 60 seconds. Esophageal contractions were measured at 5, 10 and 15 cm distal to upper esophageal sphinctm (UES). Areas of interest for saliw~ transit analysts were established in (be mouth, proximal esophagus and at the level of the distal pressure sensor'. For ea& swallow, we measured esophageal contractmns amplitude (AMP) and propagation, the time between the mouth empt~,ing to the arrival of saliva to distal esophagus (M-D), the time b-ore fbe proximal to distal esophagus (P-D) and the saliva clearauce kom the esophagus (EC). Results:Sabra transport times in upright and supine positions were not significantly &fferent. Sildenafil did not attect peristalsis in the proximal esophagus, but sigmficantly eeduced the: amplitude or even abolished contractions at 15 cm distal to UES. Abnormal distal esoptmgeal motility did not delay the arrival of saliva to the distal esophagus but prolonged the saliva clearance to the stomach in supine position Conclumon: Saliva transport to ~he distal esoptragus does not depend on gravity or normal peristalsis m (be smooth muscle esophagus, suggesting that a preserved pharyngeal pump and proximal esophageal motility might be enongh to guarantee the neutralizing saliva function a[ter acid GER" @default.
