Adult quality markers provide program accountability and informed outcomes for clinical trials. However, corresponding quality markers are lacking for children with cancer. Develop expert panel endorsed quality markers for end-of-life care for children with cancer. The Modified Delphi Panel included professional agency nominated representatives from 9 institutions and from 1) pediatric oncology, 2) adult oncology, 3) pediatric palliative care (PPC), 4) social work, 5) nursing, and 6) hospice. The panel was provided a literature review on 22 proposed quality markers derived from adult quality markers. The panel scored the importance of each on a 9-point scale, with 9 as the highest score. The panel discussed the evidence, initial scores, and their thoughts via conference call. After the call, panelists re-scored the quality metrics. A priori quality markers with median scores >7 with <2 members scoring them < 3 were considered endorsed. Sixteen indicators were endorsed including measures related to (a) avoidance of medically aggressive care (e.g., death in the ICU [Median: 8], intubation in the last 14d [7], and >2 ED visits in the last 30d [7]); (b) death in preferred location (8); (c) PPC/hospice (e.g., Involvement of PC/hospice in the last 30d of life [9] and hospice enrollment >5d before death [8]); (d) evaluation of sibling needs (7) and financial needs (7) around EOL; and (e) easing visiting hour (8), age (8), and time (8) restrictions at end-of-life. Measures related to use of chemotherapy at end-of-life (4) and home death (5) included those not endorsed by the panel. Quality markers surrounding avoiding some medically invasive care, involvement of PPC/hospice, and evaluating sibling and financial needs at end-of-life were endorsed." @default.
