Palliative care programs typically rely on referrals to identify patients. Such approaches risk inefficient use of resources through enrolling patients with minimal needs while failing to identify high need patients. Systematic case-finding using data in the electronic health record and/or administrative claims could improve on provider referral. We translated an approach to identifying patients with serious illness from the research space into a health system to enroll patients in a home-based palliative care program. The objective of this study was to analyze the yield of this approach and population characteristics. Between 4/1/2018 and 3/30/2019 we identified patients with a severe medical condition (after Kelley et al) through a query of a clinical data repository. After reviewing charts for eligibility, we emailed the provider for permission to introduce the program. We interviewed patients at enrollment to obtain data on demographics, symptoms, function, care preferences, and financial well-being. Over the 1 year period, 2457 patients were identified from the repository, 813 were ineligible at chart review. Of the remainder (n = 1644), 876 providers responded (53% response rate) and authorized the program for 562 people (64% authorization rate). To date, we have introduced the program to 270 patients and enrolled 157, an overall yield (enrolled to identified) of 6.4%. Enrolled patients were 53% female with mean age of 74.1 years. Eighty-one percent of patients had significant symptom burden and 78% reported functional impairment. Fifty-one percent definitely wanted cardiopulmonary resuscitation, 25.7% were undecided. Thirty-five percent reported financial stress. Through translation of a case-finding approach from the research space to a health system, we identified a population of patients with serious illness likely to benefit from home-based palliative care." @default.
