- W3037698970 abstract "In programmed frozen-thawed embryo transfer (FET) cycles, the endometrium is prepared prior to transfer with the use of exogenous estrogen. Serum levels of estradiol are highly variable among patient who are using exogenous estrogen, and it is unknown whether those different serum levels alter the endometrial milieu in a way that impacts pregnancy outcome in programmed FET cycles. To determine if estradiol levels prior to transfer in programmed FET cycles that were above or below the usual physiologic range (300-500 pg/mL) would impact pregnancy outcomes. Patients who underwent IVF with PGT-A with a subsequent FET in programmed cycles of a single euploid blastocyst were included in this retrospective cohort study. Serum estradiol (E2) levels were obtained on the day on starting progesterone and the day prior to transfer. Patients were stratified into three groups based on peak E2 level. The primary outcome assessed was live birth rate. Secondary outcomes included positive pregnancy rate and miscarriage rate. Patients who underwent FET in a natural cycle were excluded. Statistical analysis included chi-square test and Fisher’s exact test. P<0.05 was deemed statistically significant. A total of 335 programmed FET cycles from patients who underwent IVF with PGT-A with transfer of a single euploid blastocyst over a 4-year period were analyzed. A total of 26 patients had E2 levels less than 300 pg/mL (group A), 101 patients had E2 levels between 300-500 pg/mL (group B), and 208 patients had E2 levels greater than 500 pg/mL (group C). The live birth rate was significantly higher in group B when compared to group A and group C (63.4% in group B vs. 42.3% and 48.6% in groups A and C, respectively, p=0.03).
