- W3038863480 abstract "“How could I apply this information?” Stander use has a positive effect on bone mineral density, hip stability, and range of motion in a heterogeneous group of children including children with spinal muscular atrophy (SMA).1 Untreated children with SMA who were not walking were more likely to use standers if they had head control, were able to sit unsupported, or used respiratory support. The natural history of SMA has been well described,2,3 allowing clinicians to anticipate sequelae that might influence stander prescription and use. However, the availability of disease-modifying medication for SMA has changed the disease course and introduced new disease trajectories. Therefore, developing plans of care for children with SMA needs to be redefined. The results of this study are informative for directing stander use in chronic or untreated children. The effect of standers on scoliosis, hip subluxation, and functional prognosis is still needed in all SMA phenotypes. “What should I be mindful about when applying this information?” The revised 2018 Standard of Care Guidelines recommend a standing program 5 to 7 times a week for children with SMA types I and II based on limited evidence and expert opinion.4 While there is some evidence to support the efficacy of standing programs for children with neuromuscular disease, there is still limited information as it applies to optimal frequency and duration.1 Prescribing a standing program is a thoughtful process that should involve the entire family, health care, and education team including hospital and community-based professionals, driven by identifying and meeting specific goals. Factors that need to be considered in the present and future include the child (timing, health status, and motor development), the family (priorities and perspectives on their child), and resources (funding and space for equipment).
