Uncontrolled hyperglycemia in patients with DM significantly increases the risk for coronary microvascular dysfunction, characterized by a diminished vasodilatory capacity in response to various metabolic stimuli, which can lead to ischemia and diabetic cardiomyopathy. Although pathophysiological mechanisms that contribute to diabetic cardiomyopathy are poorly known, past studies have indicated roles for endothelial dysfunction and dysregulation of local vasodilators, particularly related to low endothelial nitric oxide production, increases in inflammatory mediators, and increased levels of reactive oxygen species.
