- W3044894307 abstract "Background: Heart failure is a state in which an abnormality of cardiac function is responsible for failure of the heart to pump blood at a rate commensurate with the requirements of the metabolizing tissues, or to do so only when filling pressures are excessively increased. It is a major public health problem and its outcome depends on amongst others, the severity of the disease. There is therefore the need for more effective and affordable means of determining disease severity and progression especially in a resource-limited country like Nigeria. Aim: This study assessed the relationship between severity of chronic heart failure and serum uric acid levels in patients at Ahmadu Bello University Teaching Hospital, (ABUTH) Zaria. Methodology: This cross-sectional descriptive study evaluated chronic heart failure patients aged 16 years and above at ABUTH and age and sex-matched controls from October 2014 to February 2015. Ethical clearance and informed consent were obtained from the Health Research Ethical Committee (HREC) of ABUTH. A structured questionnaire was completed for each successive participant and data was analyzed using statistical package for social sciences (SPSS version 16 for Windows). Results were presented on tables and charts. Statistical significance was evaluated with chi square (χ2), independent sample t-test and one-way analysis of variance (ANOVA) at ρ value of <0.05. Relationships between continuous variables were determined using Pearson’s coefficient of correlation and linear regression. Results: One hundred and eighty (180) subjects consisting of equal number of patients and controls were studied. The mean age of patients was 40.69±15.46 while for controls was 37.11±52 years. Male and female patients were 30(33.3%) and 60 (66.7%) respectively while male controls were 43 (47.8%) and female controls 47 (52.2%). There was no statistically significant difference between the mean age (ρ=0.071) and sex (ρ=0.069) of patients and controls. The prevalence of hyperuricaemia among patients and controls was 67.8% and 18.9% respectively, while their mean UA levels were 543.03µmol/L and 265.16µmol/L (ρ=0.0001). Severity of heart failure was associated with rising mean serum UA level (ρ=0.001). Left ventricular ejection fraction was significantly correlated with rising serum UA level (ρ=0.002) but E/A ratio did not show significant statistical correlation with serum UA level (ρ=0.236). Males had significantly higher UA levels than female patients. No significant statistical relationship between aetiology of heart failure and serum UA level was seen. Conclusion: Chronic heart failure is associated with hyperuricaemia in patients attending ABUTH and increases with severity of heart failure. Measurement of serum UA levels may an affordable, cheap and easily reproducible marker for assessing severity of chronic heart failure in this environment however, a longitudinal prospective study will be needed." @default.
