Various literatures reviews have been collected regarding balakarappan from siddha texts as well as modern textbooks.• 40 children with balakarappan diagnosed clinically and admitted in out patient and in patient ward were observed for clinical diagnosis, laboratory investigation and treated with Parangipattai Chooranam (int) and karappan ennai (ext).• Clinical diagnosis of balakarappan was done on the basis of clinical features describedcribed in balavagadam, athmaratchamirtham ennum vaithiya Sara sangiragam, and noinadal and noi muthal nadal thiratu.• The etiology and clinical features of karappan were correlated with the etiology and clinical features of atopic dermatitis.• Siddha system of clinical methods like enn vagai thervugal, neikuri, neerkuri were carried out in all the patients and recorded.• Routine blood and urine examination were done and find out the disease condition and safety & efficacy of the trial drug.• The trial drug Parangipattai Chooranam(int) given 2 times a day with milk and karappan ennai(ext) given for the clinical treatment & management of balakarappan the dosage of the drug is 500mg(bid) for 2-7 years, 750mg(bid) for 8-12 years.• The observation made during the clinical study showed that the trial drug Parangipattai Chooranam (int) and karappan ennai (ext) was control the disease and confirmed the clinical prognosis.• Biochemical analysis of the trial drug showed the presence of ferric iron, calcium, choride, sulphate, phosphate, sodium, magnesium.• In pharmacological analysis the trial drug had anti-inflammatory, anti-histamine, anti microbial activity etc.• The parents and children were advised to follow the preventive measures and to lead a hygienic life.CONCLUSION:All patients of balakarappan were treated with Parangipattai Chooranam (int) and karappan ennai (ext) in dosage of internal drug 500mg for 2-7 years child and 750mg for8-12 years childrens 2 times a day with milk they were treated at out patient and in patient childrens in Kuzhandhai Maruthuvam department, National Institute of Siddha, Chennai 47.• No adverse effects were noticed during the treatment period.• The trial drug Parangipattai Chooranam (int) and karappan ennai (ext) is easily available and harmlessless to infants& children.• The method of preparation is easy and the cost is comparatively ecnomical. • The drug has got anti inflammatory, anti microbial, anti-histamine, broncodilator activity, and analgesic activity.• Clinical results were found to be good in 75% of cases and moderate results were found in 20% of cases and mild results were found in 5% of cases.• The trial drug Parangipattai Chooranam (int) and karappan ennai (ext) is denoted in text book of agasthir vaithiya kaviyam and siddha vaithiya sinthamani. It is effective for all types of karappan, through this study the effectiveness of trial drug is confirmed reestablised by the author." @default.
