- W3048348030 abstract "Omitting prescription or modification of contraception in women with cancer is pitfall number one. Contraceptive choice then needs a specific riskbenefit balance in this context. Copper intra-uterine device (IUD) may be the first option in most cases, including for childless women, because it is devoid of hormonal, vascular or hepatic side effects and highly efficient. IUDs (or tubal implants) do not impair pelvic imaging quality during follow up. The main drawback of copper IUD is heavy menstrual bleeding. In womenwith non-hormone-dependent malignancies, deep vein thrombosis (DVT) risk leads the decision process, and combined contraceptives may be avoided, whatever the route of administration or composition, in some cases. Progestin-only contraceptives may then be used. In women with hormone-dependent malignancies combined contraceptives cannot be used. Progestin-only contraceptives may be discussed after prolonged follow up when other methods are not applicable. Levonorgestrel-releasing IUD has not shown beneficial effects in women with tamoxifen-induced endometrial effects, and its use remains controversial in women treated for breast cancer. Emergency contraception can be used in all cases given the very positive riskbenefit balance. Finally, general rules of contraceptive choice, information and prescriptions should of course be applied in women with malignancies." @default.
